Bridging the Gap with Case Management

My name is Charlotte Scott, and I am proud to have recently joined Circle Case Management as a Nurse Case Manager.
I am available for instruction across Cornwall and the surrounding areas.
I bring with me over a decade of nursing experience, with a particular focus on supporting people living with brain injuries and spinal injuries. Nursing, for me, has always been personal. My decision to specialise in neurological care was shaped by my own family’s experience of a brain injury. I will never forget the whirlwind of emotions we went through. There is the immediate worry – the desperate need to know that your loved one is safe, alive, and cared for – but as the days and weeks go by, you start to realise that the impact is far greater than you imagined.No one prepares you for what happens next.
Until you are in that position, you don’t truly understand that recovery from a brain injury is not just about survival or physical healing. There are changes in behaviour, emotions, and cognition. There is overwhelming fatigue. And alongside supporting the person you love, you also find yourself grieving – grieving the life you had, the person they were, and helping them to grieve too as they adjust to their new reality.
Then comes the challenge of navigating an unfamiliar system: hospitals, appointments, professionals, therapy schedules, new routines, and the daunting task of trying to understand who this new version of your loved one has become. It can feel like being dropped into an entirely new world without a map. What struck me most was the lack of continuity – once you left the hospital, it often felt like the links were broken. That ongoing support, that bridge between hospital and home, wasn’t always there.
That experience is the reason I do what I do now.
It’s why I chose nursing, why I specialised in neuro and spinal injuries, and why I am now stepping into case management with such passion and purpose.
What I Want to Be for P
When I think about P and their family starting this journey, I want to be the person who stands alongside them from the very beginning. With an Immediate Needs Assessment, I can be there right at the start, understanding their priorities, advocating for their needs, and ensuring the right services are in place. From there, I will walk with them every step of the way – through rehabilitation, through adjustment, and through the process of building a new life.What matters to me is not only helping P access therapies and care, but also helping them and their family to understand what life might look like moving forward.
I want to answer the questions I once had myself: What’s normal now? What should we worry about? What’s just part of the new baseline?
Sometimes the most powerful support is simply reassurance – helping families know what to expect and giving them permission to stop feeling like they are failing when challenges arise.My commitment is to make sure that both P and their family feel informed, understood, and supported. By really getting to know them – their hopes, their worries, their goals – I can create a rehabilitation journey that is about more than recovery. It’s about giving P the opportunity to regain independence, make choices, and live with the best possible quality of life.

Why Case Management Excites Me
Case management allows me to bring together all of my knowledge and experience and channel it directly into supporting P and their family. I see myself as a bridge builder – linking hospital teams with community services, connecting families with therapies, and pulling together all the different parts of P’s life into a joined-up, supportive network.It’s not just about coordination. It’s about making sure P has a voice, that they are heard, and that they feel empowered in decisions about their care and their future. I am there to make the system less overwhelming, to bring clarity, and to ensure that progress is not just measured in medical outcomes, but in independence, confidence, and quality of life.

I am genuinely thrilled to be part of Circle Case Management. What drew me here was the company’s reputation – award-winning services, an unwavering commitment to excellence, and above all, a client-first approach. To be surrounded by people who share my values, who understand that compassion is as important as clinical skill, makes this a perfect fit for me.I also feel privileged to be working under the guidance of Clinical Director Pippa Bird, who brings not only her leadership but also her own nursing background to the team. Having a leader who truly understands the challenges and opportunities of nursing in case management makes a world of difference. Alongside Pippa’s support, I benefit from the supervision of an experienced Senior Case Manager, whose mentorship ensures I am always developing and delivering the very best for P.And then I am also supported by the administrative team- a luxury I have never had before as a Nurse! They handle the logistics – reports, paperwork, MDT minutes, appointment scheduling, research, and so much more – which frees me to dedicate my time to P. They are incredibly knowledgeable about services, therapists, local facilities, accessible activities, and specialist providers. This creates a network of support that wraps around P and me. It means I am never working alone, and P is never facing their journey with just one person in their corner – they have an entire team working together for their success.
Looking Ahead
As I step into this role, my excitement comes from the chance to make a difference in real lives. I know from personal and professional experience how overwhelming the journey after a brain or spinal injury can be. My promise is to be present, to listen, to advocate, and to guide P and their family through every stage of rehabilitation and adjustment.My goal is not just recovery. It is about helping P build a life that feels meaningful, connected, and fulfilling. It is about ensuring they have the independence and choices they deserve, and that their family feels supported every step of the way.
