What we Do

Case managers are qualified health professionals with a background in Occupational Therapy, Physiotherapy, Psychology and Nursing. They are required to register with their respective Councils. 

Case managers need a substantial amount of clinical experience to understand not only their own role but that of other professionals and the health and social field in general. This enables Case Managers to not only understand their clients medical and rehabilitation needs, but to have a good working knowledge of rehabilitation options enabling the Case Managers advocate for their clients. 

Circle Case Management hand select their Case Managers across the U.K for their expertise, experience, and skill. They are experts in their profession and have an extensive knowledge of local services, rehab providers and therapists available.

What does a Case Manager do?

Direct Employment & Recruitment

The research, recruitment, training and supervision of care and support teams. This can be direct employment, which Circle Case Management offer HR services for, or it can be the employment of an external agency. 

Provision of Equipment and Adaptations

The Case Manager will co-ordinate and work in partnership with statutory services to ensure the client’s needs are met and monitor funding and provisions, as well as working with private therapists and services.

The Case Manager will communicate with the legal and insurance fee payers on the client’s case, ensuring the client understands the litigation process and the different stages and managing the impact on the client.

The Case Manager is able to coordinate with the education provision for clients that have special education needs. They can oversee the writing of education health care plans ( EHCP) , ensuring that children and young adults in education receive the best possible support to achieve their goals.

The Case Manager will promote client's independence enabling them to be fully included and access the community for leisure, voluntary work, and paid employment.

What is Case Management?

Case management is a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the client's health and rehabilitation needs. Put simply, our case managers bring order and stability to people’s lives, helping them to rebuild and reshape their futures following life changing injuries. 

How much does Case Management cost?

Each case varies on the level of Case Management input required but Circle Case Management will always be open and honest about any costs.
